Procedures for reissuing, changing, and adjusting information on social insurance books and health insurance cards in Vietnam is mentioned in Decision 1318/QĐ-BHXH dated September 19, 2023 announced amended and supplemented administrative procedures under the jurisdiction of Vietnam Social Insurance.

Procedure for reissuing, changing, and adjusting information on social insurance books and health insurance cards in Vietnam

The order of performing procedures for reissuing, changing, and adjusting information on social insurance books and health insurance cards in Vietnam is as follows:

Step 1. Prepare documents according to regulations.

Step 2. Submit application

(1) Participant

- People who are working: Submit documents to the unit where they work or submit them to the social insurance agency to directly collect.

- Voluntary social insurance participants, household health insurance participants, and people partially supported by the state budget for health insurance premiums: Submit documents to the service organization or social insurance agency to directly collect.

- People who are reserving the time of social insurance payment or have been granted pension or social insurance benefits and request to re-issue or adjust the content on the social insurance book: Submit to the social insurance agency nationwide.

- Participants whose health insurance is paid by the social insurance organization:

Submit your application to the Commune People's Committee or the Social Insurance Agency.

For people receiving unemployment benefits when reissuing or changing health insurance cards: Submit documents to the provincial social insurance agency.

- Participants paid by the state budget supported 100% of health insurance premiums: Subit documents to the Commune People's Committee or directly collect them from the social insurance agency.

People who have donated body parts: submit the hospital discharge certificate to the social insurance agency to directly collect.

- Pupils and students paying health insurance: Submit an application to the educational institution.

(2) Unit

- Employer unit: declare relevant documents, then submit documents to provincial and district social insurance agencies.

- Commune People's Committee; social assistance facilities; nursing facilities for war invalids and people with meritorious service;

Vocational education establishments in the fields of labor, war invalids, and social affairs; service organizations; educational facilities; and the social insurance policy department/team: declare relevant documents and then submit them to the social insurance agency.

Step 3. The social insurance agency receives the application and resolves it according to regulations.

Step 4. Receive settlement results, including a social insurance book and a health insurance card, according to the registration form.
