Guidance on Writing Personal Records of Lower Secondary School Students in Vietnam

A personal record is one of the required documents for lower secondary school enrollment. Therefore, parents need to know and guide their children on how to fill out the form correctly.

Instructions for writing personal records for secondary school students are as follows:

Page 1: COVER PAGE - STUDENT RÉSUMÉ

- Full Name: Write in uppercase with accents

Page 2: Personal Information

- Paste a 4×6 photo (taken within the last 3 months) in the top left corner.

- Full Name: Write in uppercase with accents

- Date of birth:- Ethnicity: (as per the birth certificate)

- Place of birth: (as per the birth certificate)

- Religion: Specify your religion; if none, write "none".

- Examination category: Fill in according to your exam notification; if not applicable, leave it blank.

- School code: Enter the code of the school you are enrolling in the 3 blank boxes. For example, for International University, enter QSQ.

- Examination number: Your examination number from the recent National High School Examination/aptitude test. For direct admissions or priority, leave it blank.

- Final grade results in high school: Include the results of grade 12; specify both academic and conduct categories. Skip the graduation ranking as it was removed by the Ministry of Education and Training from 2016.

- Date of joining Ho Chi Minh Communist Youth Union: According to your membership book.

- Awards and discipline: Detail any awards or disciplinary actions (if none, write "none").

- Permanent address: Exactly as in your household registration book, including house number, street, ward, district, city.

- Policy category: Specify if you belong to any policy category.

- Priority area: Specify your priority area as per your exam notification: 1; 2; 2NT; 3.

- Priority group: Specify your priority group as per your exam notification: 1, 2, 3, 4, 5, 6, 7.- Major: Indicate the major you have enrolled in; write the full name clearly.

- Entrance exam scores: Include the total score of the 3 subjects considered for admission (including bonus points, excluding priority points), and the score of each subject.

- Bonus points: If applicable, list any national or international competition points; if none, leave it blank.

- Reason for direct admission and bonus points: Detail if applicable, otherwise skip.

- National ID number: Enter your exact ID number.

- Summary of study, work, and labor history: Indicate the timeline of primary school, lower secondary school, and high school.

Page 3: FAMILY DETAILS

1. Father: Complete with father's full name, nationality, ethnicity, religion, permanent address, and contact details.

- Economic, political, social activities: Include timeline, workplace, and position if any.

2. Mother: Complete with mother's full name, nationality, ethnicity, religion, permanent address, and contact details.

- Economic, political, social activities: Include timeline, workplace, and position if any.

3. Spouse: If applicable, provide full details; if none, skip.

4. Siblings: Provide full names and current occupation and address of brothers, sisters (if any).

- Family declaration on the student’s information: Requires parent’s signature for confirmation.

- Student’s signature in the top right corner.

Complete contact details of the student and family.

What are 9 rights of lower secondary school students in Vietnam?

According to Article 35 of the Regulations on lower secondary schools, upper secondary schools and integrated schools issued under Circular 32/2020/TT-BGDDT, lower secondary school students have the following rights:

[1] Equal access to comprehensive education, guaranteed conditions of time, facilities, hygiene, and safety for learning in class and self-study at home, and provision of information on their learning and training.

[2] Utilize the school's equipment and means for learning, cultural, and sports activities.

[3] Be respected and protected, treated equally and democratically, and have the right to appeal school and educational management decisions affecting themselves.

[4] Right to transfer schools for legitimate reasons as per regulations.

[5] Right to study ahead of the prescribed age, accelerated classes, or study at an age higher than the prescribed age.

[6] Participate in activities that develop talents in subjects, sports, or arts organized by the school, if qualifying.

[7] Receive scholarships or other aid as per regulations for socially disadvantaged students, those facing life difficulties, and exceptionally talented students.

[8] Transfer schools if meeting conditions.

[9] Enjoy other rights as per regulations.

Are the educational programs at the lower secondary school level in Vietnam required to meet career orientation requirements?

According to Clause 1 and Clause 2.b, Article 30 of the Education Law 2019, the educational program for lower secondary school students must meet the following requirements:

- Ensure comprehensive, basic, systemic, and career-oriented education.

- Subjects must be relevant to real-life, suitable for the psychological age of students, and meet educational goals.

- The secondary education program should reinforce and develop the primary education content, ensuring students have basic knowledge of Vietnamese, Math, national history; other essentials of social and natural sciences, law, IT, foreign languages; and basic understanding of technology and career orientation.

Thus, one of the content requirements of the lower secondary school educational program is to meet the career orientation and systemic criteria.
