Procedure for issuing or reissuing the written confirmation of disabilities in Vietnam

Announcement of the procedure for issuing or reissuing the written confirmation of disabilities with the following main contents:

(1) Procedure:

- Step 1: When there is a need to issue or reissue the written confirmation of disabilities, the person with a disability or the legal representative of the person with a disability must submit an application according to Form No. 01 issued with Circular 01/2019/TT-BLDTBXH (amended and supplemented by Circular 08/2023/TT-BLDTBXH) and send it to the People's Committee of the commune where the person with a disability resides.

- Step 2: Within 05 working days, the Chairman of the People's Committee of the commune based on the existing records shall decide to issue or reissue the written confirmation of disabilities.

(2) Mode of implementation: To be implemented through one of the following forms: directly, online, or via public postal service.

(3) Components and number of dossiers:

- Number of dossiers: 01 set.

- Components of the dossier: An application for issuance or reissuance of the written confirmation of disabilities according to Form No. 01 issued with Circular 01/2019/TT-BLDTBXH (amended and supplemented by Circular 08/2023/TT-BLDTBXH).

(4) Requirements and conditions

- Issuance of the written confirmation of disabilities in the following cases:

+ The written confirmation of disabilities contains incorrect information compared to the Identity Card, citizen identification card, or other legally valid documents;

+ The written confirmation of disabilities is damaged and cannot be used.

- Reissuance of the written confirmation of disabilities in the following case:

+ Loss of the written confirmation of disabilities.

In the case of reissuance of the written confirmation of disabilities as specified in point a (Change in type or degree of disability) and point c (Persons with disabilities aged 6 or older, except for those with particularly severe disabilities as specified in points 1.1, 1.2, 1.5, 1.6, 1.7 of Section IV Form No. 02 issued with Circular 01/2019/TT-BLDTBXH, amended and supplemented by Circular 08/2023/TT-BLDTBXH) Clause 2 Article 8 of Circular 01/2019/TT-BLDTBXH, the procedure shall be carried out at the commune level administrative procedure (Code TTHC 1.001699) regarding the determination, redetermination of the degree of disability, and issuance of the written confirmation of disabilities.
