Instruction for Declaring C/O Form B for Exported Goods (Latest)

Circular 05/2018/TT-BCT stipulates the rules of origin and origin declaration for exported goods and imported goods.

provisions for C/O form B of Vietnam (according to the form in Appendix XI) issued for exported goods must be declared in English and typed. Please note some of the declared contents as follows:

- Box No. 1: Exporter's trading name, address, the name of the exporting country;- Box No. 2: Consignee's name, address, the name of the importing country;- Top right box: Reference number of C/O (for the authority or organization authorized to issue C/O);- Box No. 3: Departure date, name of the means of transport;- Box No. 4: Name of the authority or organization authorized to issue C/O, address, name of the country;- ...

In case multiple commodities cannot be declared on one C/O form B, use a supplementary declaration form C/O form B (according to the form in Appendix XII). The supplementary declaration must also be declared in English and typed.

Refer to detailed forms in the attached Appendices of Circular 05/2018/TT-BCT, which takes effect from April 3, 2018.
