This is the content stipulated in Decision 888/QD-BHXH amending and supplementing a number of articles in the process of collecting Social Insurance, Health Insurance, Unemployment Insurance, Occupational Accident and Disease Insurance; issuance of Social Insurance books, Health Insurance cards issued by Vietnam Social Security attached to Decision 595/QD-BHXH.

To adjust personal information (surname, given name, middle name; date of birth; gender) and to add the local code where the participant resides on the health insurance card, the health insurance participant needs to prepare one of the following documents:

- Identity Card or Citizen Identification Card or Passport: In cases where the participant has incorrectly declared compared to the original documents or the social insurance agency requests an adjustment;- In cases where the adjustment is due to an error made by the responsible unit compared to the participant's documents, the social insurance agency and that unit are responsible for reviewing and coordinating to adjust the information on the health insurance card.
