Dossier for enjoyment of lump-sum social insurance allowance for servicemen in Vietnam

According to Article 23 of the Circular No. 136/2020/TT-BQP of the Ministry of National Defense of Vietnam, dossier for enjoyment of lump-sum social insurance allowance for servicemen is in accordance with provision of Article 109 of the Law on Social Insurance of Vietnam, including:

1. The social insurance book.

2. Decision on demobilization issued by the head of a competent unit (using form issued by the Ministry of National Defense).

3. Decision on leave under the social insurance benefits issued by the head of a competent unit using form 05 issued together with the Circular No. 70/2016/TT-BQP.

4. Application for enjoyment of lump-sum social insurance allowance (form No. 14-HBQP).

5. In case an employee who is eligible for retirement suffers from one of the life-threatening diseases such as: Cancer, polio, cirrhosis ascites, leprosy, severe tuberculosis, HIV infection that has progressed to AIDS but he/she wishes to receive lump-sum social insurance allowance, he/she shall add a copy/summary of medical records showing the inability to serve his/herself; in cases of other diseases, it shall be replaced with the written record of assessment of the working capacity decrease, made by the Medical Assessment Council of competent authority in the Ministry of Defense, showing the working capacity decrease of 81% or more and and he/she cannot serve his/herself.

6. Written record of social insurance payment process (form No. 04A-HBQP).

7. Decision on enjoyment of lump-sum social insurance allowance (form No. 07D-HBQP).

8. In case the employees who have worked before January 2007 in the areas receiving the regional allowance, he/she must attach a personal statement about the time and location of service in the Army with regional allowances (form No. 04B-HBQP).

9. For persons who settle abroad, one of the following dossiers is required:

- A copy of the competent agency’s written certification of renunciation of Vietnamese nationality;

- A certified or notarized Vietnamese translation of one of the following papers:

+ Passport issued by a foreign country;

+ Visa issued by a competent foreign agency, certifying such country’s permission for entry for overseas residence;

+ Paper certifying such person is carrying out procedures for naturalization in a foreign country; paper certifying residence or permanent residence card or residence card of a term of 5 years or more, issued by a competent foreign agency.

10. In case of payment of medical assessment, the application shall be as prescribed in Clause 10 Article 9 of this Circular.
