What should be included in the personal dossier for the consideration of awarding the title of artisan?

Decree No. 123/2014/ND-CP stipulates the application dossier for the consideration of conferring the titles "People's Artisan" and "Excellent Artisan" in the field of handicrafts for individuals, including:

- Achievement declaration form for the title "People's Artisan" according to Form No. 1 and Achievement declaration form for the title "Excellent Artisan" according to Form No. 2 as specified in the appendix issued with this Decree;

- One of the documents proving contributions to the preservation, transfer of skills, or teaching: A tape, video disc, or photos describing the skills and techniques held or a verified copy in case the individual submits the dossier by mail, a photocopy (with the original for comparison) in case the individual submits directly the certificate, decision of awarding orders, medals, awards, commendation certificates or other related documents.

Additionally, the Decree also stipulates the dossier of the Provincial Council to be submitted to the Specialized Council of the Ministry, including:

- Submission letter of the Provincial Council according to Form No. 4 as specified in the appendix issued with this Decree;- List of proposed individuals for the titles "People's Artisan" and "Excellent Artisan" according to Form No. 7 as specified in the appendix issued with this Decree, accompanied by personal dossiers as stated in Clause 1 of this Article;- Minutes of the Provincial Council meeting according to Form No. 8 as specified in the appendix issued with this Decree; Ballots of Council members according to Forms No. 5, No. 6 as specified in the appendix issued with this Decree; Minutes of the community or professional association meeting for opinion gathering according to Form No. 3 as specified in the appendix issued with this Decree;- Summary of individual achievements as per the list submitted at Point b, Clause of this Article according to Form No. 9 as specified in the appendix issued with this Decree;- Decision on the establishment of the Provincial Council.
