Health insurance participants in Vietnam have the right to apply for change to the initial medical facility. The registration for change to the initial medical facility shall be carried out according to the following procedures:

1. Time for change to the initially registered medical facility for health insurance participants in Vietnam
According to Clause 2, Article 26 of the Law on Health Insurance 2008, health insurance participants in Vietnam can change their initially registered medical facilities at the beginning of each quarter, specifically in January, April, July, and October each year.
2. Application for change to the initially registered medical facility for health insurance participants in Vietnam
- According to Clause 4, Article 27 of Decision 595/QD-BHXH, when reissuing or replacing the health insurance card, the application includes:
Regarding participants
- A declaration using the specimen TK1-TS.
- If an employee wishes to enjoy more benefits from health insurance, he/she shall submit approving documents (if any) as prescribed in Appendix 03.
Employers: shall submit a declaration using the specimen D01-TS.
- Quantity of application: 1
- Submit the dossier at:
- District social insurance authority for those whose health insurance premiums are collected directly by district social insurance authority;
- Provincial social insurance authority for those whose health insurance premiums are collected directly by provincial social insurance authority.
3. Time limit for change to the initially registered medical facility for health insurance participants in Vietnam
According to Clause 2, Article 30 of Decision 595/QD-BHXH, the time limit for change to the initially registered medical facility for health insurance participants in Vietnam is no more than 03 days from the date of receipt of the complete application as prescribed.
If the participant is undergoing treatment at medical facilities, the change can be done on the same day when the complete application is received as prescribed.
4. Fee for change to the initially registered medical facility for health insurance participants in Vietnam
According to Clause 12, Article 1 of the Law on Amendments to Law on Health Insurance 2014 amending Clause 4, Article 18 of the Law on Health Insurance 2008, persons applying for reissuing the health insurance card must pay a fee. However, this regulation has been abolished by Point d, Clause 2, Article 23 of the Law on Fees and Charges 2015. Thus, health insurance participants applying for change to the initially registered medical facility are not required to a fee.
During the waiting period for the reissued card, health insurance participants still enjoy health insurance benefits according to Clause 12, Article 1 of the Law on Amendments to Law on Health Insurance 2014. However, health insurance participants must present the appointment note for the reissued card, including the information of the agency or organization, or individual authorized by the social insurance authority to receive the application for reissuing and replacing the card according to Clause 3, Article 15 of Decree 146/2018/ND-CP.
