The section "Personal History" in the Public Employee Curriculum Vitae form as stipulated in Circular 07/2019/TT-BNV regarding the policies on reporting statistics and managing public employee records shall be declared as follows:

1. Recruitment process, is a continuous process over different periods, before recruitment needs to be clearly stated:
- Time: From month, year to month, year.
- Clearly state which schools were attended, where, or what jobs were performed (including labor contracts) in which agency, organization, or unit, and where? (list any notable achievements in study or work).
2. Once recruited, clearly state:
- Recruitment time: Date, month, year of recruitment.
- Recruited into which agency, organization, or unit? (agency, organization, unit with authority to manage public employee; agency, organization, unit with authority to use public employee). What are the main job responsibilities assigned (position or job title)? The salary grade and allowance (if any).
3. Participation in political-social organizations, professional societies, etc., clearly state:
- Continuous process from month, year to month, year.
- Clearly state involvement in the Ho Chi Minh Communist Youth Union or activities in political-social organizations, political-social-professional organizations, social organizations, other social-professional organizations (domestic and foreign), etc. (name of the organization, name of the association, headquarters location, positions or titles held in these organizations).
4. Process of training and professional development, political theory, state management, foreign languages, and informatics. Only after graduating from training and development programs domestically and internationally, public employee should declare and fully record the following information criteria:
- Name of the training or development institution.
- Training or development specialty.
- Training or development time: From month/year to month/year.
- Mode of training or development: Formal, in-service, specialized, interconnected, distance education, etc.
- Degrees or certificates awarded.
5. Rewards are to record the work process and contributions of the public employee, clearly stating the following information criteria:
- Time of reward: month, year.
- Content and form of reward: reward for what achievement, form of reward such as: Certificate of Merit, Certificate of Commendation, Honor awarded by the state, Medal, Order.
- Decision-making authority for the reward.
6. Discipline public employee, clearly state:
- Time of disciplinary action: month, year.
- Reason and form of discipline such as: Reprimand, warning, relegation, demotion of professional title (demotion of official rank), dismissal, forced resignation.
- Decision-making authority for the disciplinary action.
7. Family relations of public employee record the following information:
- Relationship with the public employee such as: Father, Mother, Wife (or Husband), children, and siblings.
- Briefly summarize key characteristics of Father, Mother, Wife (or Husband), children, and siblings, including: Name, birth year, occupation, historical characteristics (whether they were arrested, imprisoned, worked for the old regime police, puppet soldiers, puppet government); current occupation (specifying position or title if any, name, and address of the workplace domestically or internationally), and registered permanent residence.
8. Family relations on the spouse's side of public employee record the following information:
- Relationship with the spouse's family of public employee such as: Father, Mother, and siblings.
- Briefly summarize key characteristics of each person with a family relationship to the spouse of public employee: Name, birth year, occupation, historical characteristics (whether they were arrested, imprisoned, worked for the old regime police, puppet soldiers, puppet government); current occupation (specifying position or title if any, name, and address of the workplace domestically or internationally), and registered permanent residence.
9. Self-assessment and evaluation of public employee
- This section is for the public employee to self-assess and evaluate their political qualities, lifestyle, disciplinary awareness, skills, and professional capacity as a basis for the organization to consider and assign the right person to the right job.
