File for Issuance and Reissuance of Caregivers and Domestic Violence Counselors Cards

Recently, the Ministry of Culture, Sports and Tourism issued Circular 23/2014/TT-BVHTTDL Amending and supplementing a number of articles of Circular 02/2010/TT-BVHTTDL Detailing the standards for counselors; issuance of counselor cards, certification of counseling practice, and training in domestic violence prevention and control.

The application for issuance, reissuance of the Care Worker Card and Counselor Card on Domestic Violence has been revised as follows:

  1. The application for issuance of the Care Worker Card and Counselor Card on Domestic Violence:

- Request form for the issuance of the Counselor Card; Request form for the issuance of the Care Worker Card;- A notarized copy of the Care or Counseling Certification on domestic violence prevention;- 02 photos sized 3 x 4 cm.- The application shall be submitted as one set, directly or via postal service to the Department of Culture, Sports, and Tourism where the Card is issued.

  1. The application for reissuance of the card includes:

- Request form for the reissuance of the Counselor Card; Request form for the reissuance of the Care Worker Card;- Old card (in case of tear or damage);- A notarized copy of the Care or Counseling Certification on domestic violence prevention.- 02 photos sized 3 x 4 cm- The application shall be submitted as one set, directly or via postal service to the Department of Culture, Sports, and Tourism where the Card is issued.

The time limit for reissuing the card is 07 working days from the date of receiving the complete and valid application, the Director of the Department of Culture, Sports, and Tourism shall reissue the Care Worker Card and Counselor Card.
