C/O form B of Vietnam issued for exported goods according to the non-preferential origin regulations as per the form stipulated in Appendix XI issued together with Circular 05/2018/TT-BCT must be declared in English and typed.
The detailed content declaration for C/O form B of Vietnam is as follows:
Box No. 1: Trade name of the exporter, address, name of the exporting country.
Box No. 2: Name of the consignee, address, name of the importing country.
Top right box: Reference number of the C/O (for the authorized agency or organization issuing the C/O).

Box No. 3: Date of departure, name of the means of transport (if sent by air, declare "by air," flight number, name of the unloading airport; if sent by sea, declare the ship's name and the unloading port's name).
Box No. 4: Name of the authority or organization authorized to issue the C/O, address, name of the country.
Box No. 5: Reserved for the customs authority at the port or place of importation.
Box No. 6: Description of goods and HS code; marks and numbers of the packages.
Box No. 7: Gross weight of the goods or other quantity.
Box No. 8: Number and date of issuance of the commercial invoice.
Box No. 9: Place of issue of the C/O, date of issue, signature, and seal of the issuing authority or organization.
Box No. 10: Place, date of application for the C/O and signature of the exporter (for the trader applying for the C/O).
Additionally, if there are multiple items that cannot be declared on the same C/O, the trader must declare a Supplemental Declaration for the C/O as stipulated in Article 9 of Circular 05/2018/TT-BCT:
Article 9. Declaration of Supplemental C/O
1. If multiple items cannot be declared on the same C/O form B of Vietnam, the trader applying for the C/O must use the Supplemental Declaration C/O form B of Vietnam according to the template specified in Appendix XII issued with this Circular. The Supplemental Declaration C/O form B must be declared in English and typed. The declaration content of the Supplemental Declaration C/O form B of Vietnam includes the same reference number as the C/O and follows the guidelines from point g to point 1, Clause 1, Article 7 of this Circular.
2. In cases where preferential origin rules under international agreements that Vietnam has signed or joined, preferential origin rules under generalized system of preferences (GSP), and other unilateral preferences of the import country for Vietnam have specific regulations on the types of Supplemental C/O form templates or specific instructions on how to declare multiple items on the same C/O, the declaration of the Supplemental C/O form shall follow the regulations guided by the Ministry of Industry and Trade for that international agreement or the regulations of the importing country.
