From October 01, 2014, the system of statistical forms for medical services applicable to provincial, district, and commune levels shall be implemented according to the forms attached to Circular 27/2014/TT-BYT, including:
- Record book for activities of providing medical services and managing health at commune-level healthcare facilities, including: 12 record book templates and 01 form (Appendix 1);- Statistical reporting forms for commune-level healthcare (hereinafter referred to as commune), including: 10 forms (Appendix 2).- Statistical reporting forms for district, urban district, district-level town, provincial city (hereinafter referred to as district), including: 16 forms (Appendix 3).- Reporting forms for province and centrally-run cities (hereinafter referred to as province), including: 18 forms (Appendix 4).
Additionally, Circular 27/2014/TT-BYT stipulates that the Department of Health is responsible for assigning one district-level healthcare unit as the focal point for implementing statistical reporting policies for all healthcare activities within the district.
The Planning and Finance Department, Ministry of Health, is responsible for guiding, inspecting, and supervising the implementation of recording forms, methods of data collection, and reporting aggregation to ensure uniform implementation nationwide.
