Application for Support When a Mediator Encounters an Accident During Mediation

According to Decree 15/2014/ND-CP, when a mediator encounters an accident or risk affecting their health or life while performing mediation activities, a dossier should be prepared to request support. Specifically, the dossier includes:

- A support request letter from the mediator or the mediator's family in case the mediator suffers a loss of life, confirmed by the head of the mediation group or the Head of the Front Work Committee in case the harmed mediator is the head of the mediation group;- A verification report on the condition of the mediator suffering from the accident, confirmed by the People's Committee at the commune level where the accident occurred or by the police agency where the accident occurred;- Hospital discharge papers, invoices for payment of medical examination and treatment costs;- Valid documents on actual income in terms of monthly salary or wage of the person who suffered the accident, confirmed by the employing organization or individual, to allow the competent state agency to determine the actual lost or reduced income. This includes: labor contract, decision on salary increase by the employing organization or individual, or a statement of the actual income of the mediator suffering the accident, and other documents proving legitimate actual income;- Death certificate in case the mediator suffers a loss of life (original or certified copy if sent by post; photocopy and original for comparison if submitted directly).
