Application for Submitting documentary materials of intangible cultural heritage, relics, antiquities, and national treasures to the Museum in Vietnam (Internet image)

According to Article 5 of Circular 20/2012/TT-BVHTTDL, it is stipulated that the documentation for submitting intangible cultural heritage materials, relics, antiquities, and national treasures to public museums or competent state authorities shall be prepared by the sender; with the components as follows:

Regarding the documentation for submitting intangible cultural heritage materials:

- Application for submitting documents, artifacts (for organizations), according to Form No. 1 issued together with this Circular; Application for submitting documents, artifacts (for individuals), according to Form No. 2 issued together with this Circular;

- A copy of the valid ID card (for Vietnamese individuals), a copy of the valid passport (for individuals who are overseas Vietnamese and foreigners), a copy of the document proving legal entity status (for organizations); in case of direct submission, bring the original documents for comparison; in case of postal submission, the copies must be notarized or certified;

- Information sheet on intangible cultural heritage materials, according to Form No. 3A issued together with this Circular;

- A copy of the inventory sheet of intangible cultural heritage or a copy of the certified or notarized list of intangible cultural heritage inventory kept in documents, artifacts (for intangible cultural heritage already inventoried);

- A notarized or certified power of attorney for submitting intangible cultural heritage documents (in case the sender, who is an individual, mandates another person to submit intangible cultural heritage documents).

Regarding the documentation for submitting relics, antiquities, and national treasures:

- Application for submitting documents, artifacts (for organizations), according to Form No. 1 issued together with this Circular; Application for submitting documents, artifacts (for individuals), according to Form No. 2 issued together with this Circular;

- A copy of the valid ID card (for Vietnamese individuals), a copy of the valid passport (for individuals who are overseas Vietnamese and foreigners), a copy of the document proving legal entity status (for organizations); in case of direct submission, bring the original documents for comparison; in case of postal submission, the copies must be notarized or certified;

- Information sheet on artifacts: each artifact, antique, national treasure shall have one (1) sheet, according to Form No. 3B issued together with this Circular, accompanied by at least 2 photos of the artifacts, size 10cm x 15cm (color photos, 1 general photo, 1 photo detailing the most important feature of the artifact, with a scale ruler), ensuring adequate basis for identifying the artifact;

- A notarized or certified copy of the document certifying the artifact has been inspected, registered, or recognized (for national treasures);

- A notarized or certified power of attorney for submitting documents, artifacts (in case the sender, who is an individual, mandates another person to submit documents, artifacts).

More details can be found in Circular 20/2012/TT-BVHTTDL, which comes into effect in Vietnam from  February 15, 2013.
