5 Steps for Managing Individuals Receiving Social Work Services

Circular 02/2020/TT-BLDTBXH stipulates the management of individuals receiving social work services provided by social assistance facilities both at the facility and at the commune, ward, commune-level town level, to be carried out according to a 5-step process:

  1. Collect information and assess the care and support needs of the individual.
  2. Develop a care and support plan for the individual.
  3. Implement the care and support plan for the individual.
  4. Monitor, review, and adjust the care and support plan for the individual.
  5. Evaluate and conclude the management of the individual.

For instance, the manager of the individual is responsible for collecting information related to the individual, including:

First, the individual's information

- Basic information, including: Full name, date of birth, gender, marital status, residential address, contact information, personal identification number, ID card number, or citizen identification card number (if any);- Educational level and professional qualifications;- School;- Occupation;- Income of the individual;- Social services and policies that the individual is benefiting from;- Support needs in order of priority of the individual;

Second, the individual's health information

- Illnesses and causes or type/degree of disability;- Characteristics of illnesses/disabilities;- Labor ability;- Ability to self-care in daily activities;- Health treatment and rehabilitation process and results before entering the social assistance facility (if any);- Current physical, mental, and psychological status.

Third, information about the individual's family

- Full name of the household head, date of birth, residential address, contact information, personal identification number, ID card number, or citizen identification card number (if any);- Relationship with the individual;- Main job of the family;- Number of family members;- Position of the individual in the family;- Economic condition;- Main sources of the family's income, including earnings from work, monthly social assistance policies, and other social assistance programs;- Expenses for food, clothing, school fees, medical examination and treatment, medicine, and other expenses;- Housing and living conditions;- The family's ability to care for the individual;- Monthly social allowance and basic social services;- Support needs in order of priority;- Other information (if any).

Fourth, information about the guardian or caregiver (if any): Full name, date of birth, gender, marital status, residential address, contact information, personal identification number, ID card number, or citizen identification card number (if any).
