* This form is used to apply remedial measures in case of not issuing a decision on administrative penalties as specified in Article 65 of the Law on Handling Administrative Violations or to enforce a decision on administrative penalties in case the penalized person dies, goes missing, or the penalized organization dissolves, goes bankrupt as specified in Article 75 of the Law on Handling Administrative Violations.

In the case where the decision maker is the Chairman of the People's Committee, the agency name shall be written according to the guidance on the format stipulated in Decree No 30/2020/ND-CP dated March 5, 2020 of the Government of Vietnam on clerical work.

In cases where the violating entity cannot be identified, state "Cannot identify the violating entity" and Clause 1, Section 1, Article 1 of the Decision.
In cases where the individual dies, goes missing, or the organization dissolves, goes bankrupt without any individual or organization to transfer the rights and obligations, state "Individual dies, goes missing/organization dissolves, goes bankrupt without any individual or organization to transfer the rights and obligations" and Clause 1 Article 1 of the Decision.

^(9)^ In urgent cases requiring immediate remediation to promptly protect the environment, ensure traffic, the state agency that has organized the implementation of remedial measures according to Clause 1 Article 85 of the Law on Handling Administrative Violations, shall state the name of the agency that has carried out the remedial measures.
