Application for Issuance of Certificate of Eligibility for Fire Prevention and Fighting Service Business

According to Clause 1, Article 45 of Decree 136/2020/ND-CP (Effective from January 10, 2021), the content is stipulated as follows:

Application dossier for a Certificate of Eligibility for Fire Prevention and Fighting Service Business includes:

- A written request for a Certificate of Eligibility for Fire Prevention and Fighting Service Business (Form No. PC33);

- Diploma or certificate of training in fire prevention and fighting knowledge of the head or the legal representative of the business establishment;

- A list of individuals with Certificates of Practice for consulting on fire prevention and fighting suitable to the business field of the establishment; accompanied by Certificates of Practice for consulting on fire prevention and fighting and recruitment decision or labor contract of each individual;

- Documents proving the condition of facilities, means, and equipment ensuring business activities: Certificate of ownership or rental contract of the operation site; a list of means and equipment for business activities; a certificate of quality recognition of the laboratory and an evaluation calibration certificate of inspection equipment from the competent authority for the case of requesting a Certificate of Eligibility for fire prevention and fighting technical inspection, testing consulting services.
