Application for Inspection and Issuance of Fire Prevention and Fighting Equipment Inspection Certificate
According to Clause 5, Article 38 of Decree 136/2020/ND-CP (Effective from January 10, 2021), the following provisions are stipulated:
Application dossier for inspection and issuance of the Fire Prevention and Fighting Equipment Inspection Certificate:
- The application dossier for inspection and issuance of the Fire Prevention and Fighting Equipment Inspection Certificate: Proposal document for inspection and issuance of the Fire Prevention and Fighting Equipment Inspection Certificate (Form number PC26) from the unit directly engaged in domestic production, assembly, modification, or import of fire prevention and fighting equipment; Certificate of origin or factory release certificate of the equipment; Quality certificate of the equipment (if any); technical documents of the equipment subject to inspection;
- The application dossier for fire prevention and fighting equipment inspection: Proposal document for fire prevention and fighting equipment inspection (Form number PC26) from the unit directly engaged in domestic production, assembly, modification, or import of fire prevention and fighting equipment; Certificate of origin or factory release certificate of the equipment; Quality certificate of the equipment (if any); technical documents of the equipment subject to inspection;
- The application dossier for the issuance of the Fire Prevention and Fighting Equipment Inspection Certificate: Proposal document for the issuance of the Fire Prevention and Fighting Equipment Inspection Certificate (Form number PC27); inspection report of the fire prevention and fighting equipment from the business establishment providing consulting, inspection, and technical inspection services for fire prevention and fighting; sampling report of the equipment for inspection (Form number PC28); Certificate of origin or factory release certificate of the equipment; Quality certificate of the equipment (if any); technical documents of the equipment subject to inspection;
- Documents and papers in the dossier must be original or certified copies, notarized, or accompanied by originals for comparison by the dossier receiving officer. If the dossier is in a foreign language, it must be accompanied by a Vietnamese translation, and the requesting agency, organization, or individual must be responsible for the content of the translation.
