a) Description of the scale and function of the work: the main technical parameters, the main functions of the parts or work items;

b) Assessment of the suitability of the work's scale and functions with the construction permit (for works requiring a construction permit), construction design, technical instructions, construction methods, approved safety measures and technical regulations and standards applicable to the works.

2. Assessment of the suitability of the construction contractor's capacity with the bid dossier and construction contract.

3. Evaluation of completed work volume and progress, construction organization and assurance of labor safety in construction work.

4. Evaluation of testing and inspection of construction materials, products, components and equipment installed in the works according to the approved test plan.

5. Evaluation of the organization and results of inspection, observation and control experiment (if any).

6. Evaluation of the organization of the acceptance test of construction work, phase acceptance test (if any).

7. Design of changes and the appraisal and approval of the adjusted design during the construction process (if any).

8. Shortcomings, defects in quality, work incidents in the course of work construction (if any) and assessment of causes, measures and remedial results according to regulations.

9. Assessment of conformity of quality management records as prescribed.

10. Assessment of compliance with regulations of law on environment, law on fire prevention and fighting and other relevant laws (if any).

11. Assessment of the conformity of the operation process, the construction maintenance process according to regulations.

12. Evaluation of conditions for acceptance of completed bidding packages, stages, work items and construction works.

SAMPLE PERIODICAL REPORT ON CONSTRUCTION SUPERVISION OF WORKS/WORK ITEMS

1. Assessment of the suitability of the work's scale and functions with the construction permit (for works requiring a construction permit), construction design, technical instructions, construction methods, and measures. approved safety measures and technical regulations and standards applicable to the works.

2. Assessment of the suitability of capacity of construction contractors compared with bid documents and construction contracts:

a) Name of the construction unit;

b) Assessment of the suitability of the capacity of the project leader or the contractor's project manager, the person in charge of construction techniques directly compared with the construction contract regulations and the provisions of law;

c) Summary of statistics and evaluation of the conformity of machinery and equipment for construction in the reporting period compared with the construction contract.

3. Evaluation of the volume and progress of work completed in the reporting period, the organization of construction and assurance of labor safety in the construction of works:

a) Work volume completed in the reporting period. Workload has been accepted. Compare with the overall construction progress and cause of delay (if any);

b) Evaluation of construction organization compared with approved construction methods. Changes in construction methods (if any);

c) Evaluation of the implementation of the contents of the approved safety assurance plan.

4. Statistics of experimental work performed in the reporting period on the number of experimental results for each type of experiment. Evaluate the quality control of testing and inspection of materials, construction products, components and equipment installed in the works according to the approved test plan.

5. Statistics of construction works tested and accepted in the reporting period, the pre-acceptance work of the period (if any).

6. Statistics of design changes on the construction site during the reporting period.

7. Statistics of shortcomings, defects in quality, construction incidents (4) in the reporting period (if any); shortcomings and defects in quality have been remedied during the reporting period. Assessment of causes, measures and remedial results according to regulations.

8. Proposals, recommendations on progress, personnel, design and other technical issues.

Note: 

(1) Name of the organization performing construction supervision.

(4) In case there is a construction incident in the reporting period, the report shall be enclosed with a dossier of work incident settlement as prescribed.

Notes :

(1) Name of the investor.

(2) Name of the state management agency in charge of construction g in the locality where the work is to be built.

(3) Name of the specialized construction agency that inspects the pre-acceptance test of the construction work as prescribed in Clause 2, Article 24 of this Decree in case the works are subject to the pre-acceptance inspection as prescribed in Clause 1, Article 24 of this Decree.

(4) The cases specified at Points b, e, h and i, Clause 2, Article 89 of Law No. 50/2014/QH 13 are amended and supplemented in Clause 30, Article 1 of Law No. 62/2020/ QH14 enclosed with construction design documents; In the case specified at Point g, Clause 2, Article 89 of Law No. 50/2014/QH13, as amended and supplemented in Clause 30, Article 1 of Law No. 62/2020/QH14, the attached dossier includes: design dossier construction and documents proving the satisfaction of conditions for construction permits.
