Application for compulsory rehabilitation in rehabilitation centers for Vietnamese residents
Based on Clause 1, Article 41 of Decree 116/2021/ND-CP, which stipulates the application for compulsory rehabilitation in rehabilitation centers for Vietnamese residents as follows:
- For drug addicts with stable residences:
a) 01 summary of the personal background of the person proposed to be sent to a compulsory rehabilitation in rehabilitation centers center according to Form No. 30 Appendix II of this Decree;
b) 01 record of violations of the law on registration for rehabilitation, voluntary rehabilitation at home, in the community, post-rehabilitation management, and treatment of opioid addiction using substitute medication according to Form No. 27 Appendix II of this Decree;
c) 01 copy of the result slip determining the drug addiction status issued by the competent authority;
d) 01 written statement from the person proposed to be sent to a compulsory rehabilitation in rehabilitation centers center according to Form No. 31 Appendix II of this Decree;
dd) The Chairman of the communal-level People's Committee or the Police agency shall prepare the dossier to request the compulsory rehabilitation in rehabilitation centers measure by making a proposal letter according to Form No. 29 Appendix II of this Decree, accompanied by the documents specified at points a, b, c, and d, Clause 1 of this Article, and any other relevant documents (if any).
Application for compulsory rehabilitation in rehabilitation centers for Vietnamese non-residents
Based on Clause 2, Article 41 of the above-mentioned Decree, which stipulates the application for compulsory rehabilitation in rehabilitation centers for Vietnamese non-residents as follows:
- For drug addicts without stable residences:
a) The documents specified in Clause 1 of this Article;
b) A document from the competent Police authority confirming that the drug addict does not have a stable residence.
