Notes:
At the end of each page of the ledger, record the total quantity and weight for that page and have the timber owner sign; at the end of the ledger, record the total quantity, and weight for each type of timber throughout the ledger.
(1) The number of the timber ledger is recorded by the local Forestry Agency for tracking and validation in cases where timber requires verification or approval, alternatively, the timber owner assigns a serial number based on the export timber ledgers made in the year for cases where timber does not require verification or confirmation by the Forestry Agency before export. Record as follows: 20/001: 20 is the year 2020; 001 is the serial number of the ledger created.
(2) Page number of the ledger: If the ledger has more than one page, the declarant must specify the page number and the total number of pages for verification and inspection by the competent authorities.
(3) Record the name in Vietnamese or the English transaction name (if any) for organizations/full name for individuals.
(4) Specify the business registration number or enterprise code or tax code for organizations / ID card number or citizen ID card number for individuals.
(5) Specify the address as in the business registration for organizations / permanent address as on the ID card or citizen ID card for individuals.
(6) Record the name in Vietnamese or the English transaction name (if any) for organizations or full name for individuals.
(7) Based on the source of export timber, the timber owner ticks the appropriate boxes regarding the timber's origin.
(8) Specify if the timber species is listed in CITES Appendices (Appendix I, Appendix II) or in the List of endangered, precious, rare species (Group IA, Group IIA) or common species (TT).
(9) Applicable for timber subject to verification according to Clause 1, Article 9 of this Decree; the local Forestry Agency validates the content declared by the timber owner.
