1. (*): Record as administrative violation record if it is a management book, tracking administrative violation record form; decision on sanctioning administrative violations if it is a management book, tracking the form of decision on sanctioning administrative violations.
- Column (4) and column (8): Record the number of sheets and list them by serial number. For example: 100 sheets, with serial numbers from 00001 to 00100; 100 sheets, with serial numbers from 00001 to 00050 and from 00101 to 00150.
- Column (5): The person entering/managing the form of the unit signs and confirms.
- Column (11) and column (12): Record the number of volumes, number of sheets and list by serial number. For example: 02 volumes, 100 sheets, with serial numbers from 00001 to 00100; 02 volumes, 100 sheets, with serial numbers from 00001 to 00050 and from 00101 to 00150.
- Column (13): Record the number of sheets and details by serial number. For example: 03 sheets, including series 00001, 00035, 00070.
- Section "Monitoring form usage": Monitor and compare data on a monthly basis. For agencies and units that use a small number of forms, it is possible to monitor and compare data on a quarterly basis.
2. Agencies and units can add additional content according to the internal management regulations of the agency or unit.
