How to fill out the Marriage Registration Annotation Declaration Form:
(1) Clearly indicate the name of the agency performing the marriage registration annotation.
(2) Only fill out in cases where the individual requesting civil registration does not have/does not provide a personal identification number, citizen identity card, ID card, or other valid identification.
If information on “Residence” is required, it should be recorded according to the permanent residence; if no permanent residence is available, record according to the temporary residence; if there is no permanent or temporary residence, record according to the current place of residence.
(3) Enter the personal identification number or citizen identity card number (e.g., Citizen Identity Card number 025188001010 issued by the Department of Administrative Management of Social Order on November 20, 2021).
If there is no personal identification number or citizen identity card, record the valid replacement document (passport, ID card, etc.).
(4) Indicate the name of the agency, country where the marriage was registered, the type of document proving the marriage issued by the competent foreign authority, the number (if any) and the date of issuance.
Example: Marriage registration in the Republic of France, Marriage Certificate No. 0A3456 issued on March 08, 2016.
(5) If never married, clearly state “Never married.” If previously married, clearly state the basis for the termination of the marriage. If the basis for termination of the marriage is divorce, clearly state the type of divorce document, effective date, and issuing authority.
(6) Tick the box if a copy is requested and specify the quantity.

What Conditions Must Be Met to Annotate Marriages in Vietnam?
According to Article 34 of Decree 123/2015/ND-CP, conditions for recording the marriage of Vietnamese citizens (i.e., marriage annotation) resolved at a competent foreign authority abroad are as follows:
- The marriage between Vietnamese citizens or between a Vietnamese citizen and a foreigner resolved by a competent foreign authority abroad is recorded in the civil status register if at the time of the marriage, both parties meet the marriage conditions and do not violate prohibitions according to the Law on Marriage and Family of Vietnam 2014.
- If at the time of registration at the competent foreign authority, the marriage does not meet the marriage conditions but does not violate prohibitions according to the Law on Marriage and Family of Vietnam 2014, and at the time of requesting to record the marriage in the civil status register, the consequences have been remedied or the marriage annotation is to protect the rights of Vietnamese citizens and children, the marriage is also recorded in the civil status register.
What Documents Are Required for Marriage Registration Annotation?
According to Clause 1, Article 35 of Decree 123/2015/ND-CP, the procedure for marriage registration annotation is as follows:

- The marriage registration annotation dossier submitted by one of the marrying parties to the competent authority as stipulated in Clause 1, Article 48 of the Law on Civil Status, includes the following documents:
a) Declaration form as prescribed;
b) A copy of the marriage certificate issued by the foreign competent authority;
c) In addition to the documents stipulated in Points a and b of this Clause, if the dossier is sent via postal system:
+ A copy of the identification documents of both male and female parties as stipulated in Clause 1, Article 2 of this Decree must be submitted;
+ For Vietnamese citizens who have divorced or annulled the marriage at a competent foreign authority, a copy of the record of the divorce or marital annulment must be submitted according to Clause 2, Article 37 of this Decree.

The marriage registration annotation dossier submitted by one of the marrying parties to the competent authority as stipulated in Clause 1, Article 48 of the Law on Civil Status 2014, includes the following documents:
- Declaration form as prescribed;
- A copy of the marriage certificate issued by the foreign competent authority;
- In addition to the documents stipulated in Points a and b, Clause 1, Article 35 of Decree 123/2015/ND-CP, if sending the dossier via postal system:
+ A copy of the identification documents of both male and female parties as stipulated in Clause 1, Article 2 of Decree 123/2015/ND-CP must be submitted;
+ If the Vietnamese citizen has divorced or annulled the marriage at a competent foreign authority, a copy of the record of the divorce or marital annulment must be submitted according to Clause 2, Article 37 of Decree 123/2015/ND-CP.
