Recently, the Ministry of Home Affairs of Vietnam issued Circular No. 04/2020/TT-BNV on elaborating to Decree No. 93/2019/ND-CP dated November 25, 2019 of Vietnam’s Government on organization and operation of social funds and charitable funds.

Annex I issued together with Circular No. 04/2020/TT-BNV of the Ministry of Home Affairs of Vietnam (effective from December 01, 2020) stipulates document forms for social funds and charitable funds (hereinafter collectively referred to as “funds”) and individuals and organizations related to organization and operation of funds. To be specific:

Form 1.1	Application for issue of license of establishment and recognition of Fund Charter
Form 1.2	Sample charter of social fund
Form 1.3	Sample charter of charitable fund
Form 1.4	Commitment to contributed property for fund establishment
Form 1.5	Minutes of the election of positions within the Fund's founding board
Form 1.6	Written request for extension of time to transfer the ownership of contributed property
Form 1.7	Application for change of license of establishment and recognition of Fund Charter (amended and added)
Form 1.8	Application for issue of license of establishment and recognition of Fund Charter on the basis of expanding the scope of operations
Form 1.9	Announcement of establishment of branch (representative office) of the Fund
Form 1.10	Application for fund dissolution
Form 1.11	Application for fund renaming and recognition of Fund Charter
Form 1.12	Application for fund separation
Form 1.13	Application for fund division
Form 1.14	Application for fund consolidation
Form 1.15	Application for fund merger
Form 1.16	Application for resume of fund operation
Form 1.17	Report con correction of the fund’s mistake
