This is one of the two forms recently supplemented under Circular 41/2019/TT-BCA issued by the Ministry of Public Security, amending and supplementing certain provisions of Circular 66/2015/TT-BCA on forms used in the issuance, management of citizen identification cards, the citizen identification archives, and the National Population Database.
Currently, the minutes for the handover and receipt of documents for the issuance, renewal, and reissuance of Citizen Identity Cards using Form CC12 are used for the unit handling the procedure for issuance, renewal, and reissuance of Citizen Identity Cards to transfer the dossier for issuance, renewal, and reissuance of Citizen Identity Cards to the archive management unit under the Department of Administrative Management on Social Order of the Provincial or Municipal Police under the Central Government. The unit receiving the dossier for issuance, renewal, and reissuance of Citizen Identity Cards is responsible for checking and fully receiving the dossier as prescribed.

To be specific, the manner of recording information in the minutes for handover and receipt of documents for issuance, renewal, and reissuance of Citizen Identity Cards (CC12) is stipulated in Circular 41/2019/TT-BCA as follows:
- Section “at ....hour...., date, month, year”: Fully record the hour, date, month, year of handing over the dossier; “at”: Record according to the location where the dossier is handed over;- Section “representative of the handing side” and “representative of the receiving side”: Fully record the full name, rank, position, and unit of work of the officers from the unit handing over and the unit receiving the dossier;- Section “Total number of dossiers”: Fully record the total number of dossiers handed over; including fully recording “Total number of new issuance dossiers”, “Total number of renewal dossiers”, and “Total number of reissuance dossiers”.- Section “With the list attached from dossier number....to dossier number...”: Record the serial number according to the handover dossier list.
Especially: For the sections “Receiving side”, “Handing over side”:
- In the case of handing over the dossier within the Department of Administrative Management on Social Order, the team leader is responsible for signing to confirm in the handover minutes. Officers from the handing side and receiving side must sign and clearly record their full names in the handover and receipt minutes.- In the case of handing over the dossier between the district police and the Department of Administrative Management on Social Order, the district police leader and the leader of the Department of Administrative Management on Social Order must sign and seal the handover minutes. For dossiers issued at the National Citizen Identity Center under the Department of Administrative Management on Social Order, the leader of the National Citizen Identity Center and the leader of the Department of Administrative Management on Social Order receiving the dossier must sign the handover. Officers from the handing side and receiving side must sign and clearly record their full names in the handover and receipt minutes.
