(1) General information about the educational institution

- Name of the educational institution (including the name in a foreign language, if any).

- Address of the main office and other operating addresses of the institution, phone number, email address, electronic portal, or website of the institution (hereafter referred to as the electronic portal).

- Type of educational institution, directly managing organization/authority or owner; name of the investor establishing the institution, country/territory (for institutions with foreign investment); list of organizations and individuals contributing capital (if any).

- Mission, vision, goals of the institution.

- Summary of the establishment and development history of the institution.

- Information about the legal representative or spokesperson or contact representative, including: Full name, position, workplace address; phone number, email address.

- Organizational structure:

+ Decisions on establishment, permit of establishment, merger, division, separation, permit for educational activities, suspension of educational activities of the institution or its branch (if any);

+ Decision on recognition of the school council, president of the school council, and list of school council members;

+ Decision on transfer, appointment, recognition of the principal, vice-principal, director, deputy director, or head, deputy head of the institution;

+ Organizational and operational regulations of the institution; functions, tasks, and powers of the institution and its affiliated, subordinate, and member units (if any); organizational chart of the institution;

+ Decision on establishment, merger, division, dissolution of affiliated, subordinate, and member units (if any);

+ Full name, position, phone number, email address, workplace address, tasks, and responsibilities of the institution's leaders and the leaders of its affiliated, subordinate, and member units (if any).

- Other documents of the institution: Development strategy of the institution; democratic regulations at the institution; resolutions of the school council; regulations on administrative, human resource, and financial management; policies to attract and develop the teaching staff and education management officials; recruitment plans and announcements of the institution and other internal regulations, if any.

(2) Financial income and expenditure

- The financial situation of the institution in the previous financial year immediately preceding the reporting time, in accordance with legal regulations, including the structure of income and expenditure as follows:

+ Income categorized by: Source of funds (state budget, investor support; tuition fees, fees, and other collections from learners; sponsorship funds and contractual income; other income) and type of activity (education and training; science and technology; other activities);

+ Expenditure categorized by: Salary and income expenses (salary, allowance, additional salary, and other salary-like expenses for teachers, lecturers, management officials, staff,...); expenses for facilities and services (procurement, maintenance, repair, operation of facilities, equipment, rental services directly serving educational, training activities, research, staff development,...); student support expenses (scholarships, allowances, living support, artistic, competitive activities,...); other expenses.

- Tuition and fee amounts collected from learners, including: tuition fees, fees, all other collections and amounts besides tuition fees, fees (if any) in the academic year and projections for each subsequent academic year of the school level or course before admissions or recruitment.

- Annual policy implementation results and policies on subsidies and exemptions, reductions of tuition fees, scholarships for learners.

- Balance of funds according to the regulations, including special-purpose funds (if any).

- Other financial disclosure content implemented according to legal regulations on finance, budget, accounting, auditing, and grassroots democracy.
