According to Circular 55/2015/TT-BLDTBXH, the application for receiving and managing minors who are subjected to educational measures at communes, wards, towns at child assistance facilities includes the following documents:

- Written request from the Chairman of the competent commune-level People's Committee who decides on the application of educational measures at communes, wards, towns; - Decision not to apply educational measures at communes, wards, towns or certificate of completion of educational measures at communes, wards, towns (if any); - Application from the subject or guardian requesting the facility to accept the subject as specified in Point b, Clause 1, Article 2; - A copy of the identity card or personal identification documents of the subject (if any); - Handover minutes of the subject by the commune-level People's Committee and the facility; - A copy of the Medical Record (if any); - Decision of acceptance from the competent state authority. |
- Admission application of the subject into the facility; - Plan for management, education, and assistance of the subject; - Decision to return the subject to the family, community; - Other documents related to the subject. Note: The facility is responsible for creating the management application of the subject. |
