Dossier for Registration and Declaration of Organizations and Individuals Qualified to Provide Voluntary Drug Detoxification Services
Based on Clause 1, Article 17 of Decree 116/2021/ND-CP stipulating the dossier for registration and declaration of organizations and individuals qualified to provide voluntary drug detoxification services at home and in the community as follows:
- The dossier for registration to provide voluntary drug detoxification services at home and in the community includes:
a) A document from the organization or individual requesting the declaration of qualifications to provide voluntary drug detoxification services at home and in the community according to Form No. 13, Appendix II of this Decree;
b) 01 copy of the establishment decision, establishment permission, or business registration certificate (for service providing organizations);
c) Documents proving the fulfillment of the necessary physical infrastructure and equipment conditions for providing services;
d) 01 original list of employees according to Form No. 03, Appendix II, accompanied by 01 copy of degrees, certificates, and criminal record certificates issued within no more than 03 months from the date of submission of the dossier for each employee; a brief curriculum vitae of the head or the legal representative of the service-providing facility according to Form No. 04, Appendix II of this Decree;
e) The proposed process of providing voluntary drug detoxification services at home and in the community.
Procedure for Registration and Declaration of Organizations and Individuals Qualified to Provide Voluntary Drug Detoxification Services
Based on Clause 2, Article 17 of the aforementioned Decree stipulating the procedure for registration and declaration of organizations and individuals qualified to provide voluntary drug detoxification services at home and in the community as follows:
- Implementation procedure:
a) Organizations and individuals directly, via post or electronic means, submit 01 dossier according to Clause 1 of this Article to the District-level People's Committee where the organization is headquartered or where the individual resides. In the case of electronic submission, the organization or individual is responsible for retaining all original documents and is accountable before the law for the accuracy and truthfulness of the dossier.
b) Within 10 working days from the date of receipt of a valid registration dossier, the Chairman of the District-level People's Committee shall organize the appraisal and declaration of organizations and individuals qualified to provide voluntary drug detoxification services at home and in the community according to Form No. 14, Appendix II of this Decree. In cases where the declaration conditions are not met, a written response stating the reasons must be provided.
