What are instructions for preparing the population report by healthcare facilities on the list of pregnant women screened and diagnosed prenatally by quarter in Vietnam?
Appendix III also provides guidelines for preparing the specialized population report by healthcare facilities on the list of pregnant women screened and diagnosed prenatally (Form 02-DS/CSYT) as follows:
Purpose: To reflect the results of the prenatal screening and diagnosis services provided by the healthcare facility. Evaluate the effectiveness of communication and advocacy efforts to encourage people to undergo prenatal screening and diagnosis services.
Reporting period: This report is prepared on a quarterly basis.
Compilation and recording:
Write the name of the locality (IN CAPITAL LETTERS) on the top right of the form, along with the reporting year.
In case the healthcare facility has a code, write the code and name of the facility.
(1) Health Insurance Card Number: Write the health insurance card number of the woman (if available).
(2) Full name: Write the surname, middle name, and given name of the woman in CAPITAL LETTERS.
(3) Place of residence (province, district, commune, specific address): Write the current residential address of the woman.
(4) Date of birth: Write the date of birth in the format DD/MM/YYYY.
(5) Gestational age: Write the number of months of pregnancy for the woman.
(6) Date of service: Write the date of prenatal screening and diagnosis in the format DD/MM/YYYY.
(7) Screening and diagnostic results: Write the screening and diagnostic results (normal, at risk) as per Decision No. 1807/QD-BYT dated April 21, 2020, issued by the Minister of Health on guidelines for technical expertise in prenatal screening, diagnosis, treatment, and care for newborns.
(8) Notes: Include the number of households and write "continue to be monitored, managed in the community" for cases with a conclusion of "at risk".
