Under Appendix I attached to Decree 102/2020/ND-CP, Form No. 14 - Report on timber import and export in Vietnam is specified as follows:

Notes:
(1) Applies to both ad-hoc and periodic reports.
(2) State the commercial name of the Timber species for each species and track according to the invoice numbers when importing and exporting Timber.
(3) Clearly state whether the Timber is sourced from natural forest (RTN), plantation (RTR), Timber after confiscation processing (TT), or imported Timber (NK).
(4) Indicate whether the Timber belongs to the CITES Appendices (PLI, PLII) or the List of endangered, precious, rare species (IA, IIA) or common species (TT).
