What documents are included in the application for a pharmaceutical practice certificate in Vietnam?
Pursuant to Article 24 of Law on Pharmacy 2016 (amended by Article 1 of Law on Pharmacy Amendment 2024), the application for a pharmaceutical practice license must include the following documents:
[1] Application form for a pharmaceutical practice license with a portrait photo of the applicant taken within no more than 6 months.
[2] Professional qualification diploma.
[3] Health certificate for practicing pharmacy issued by a competent medical facility.
[4] Certificate of professional practice duration issued by the head of the facility where the applicant practiced.
[5] Certificate of completion of training program, updating professional knowledge about pharmacy for cases where the pharmaceutical practice license was revoked and the practitioner does not have a certificate of completing the training program, updating professional knowledge about pharmacy within 3 years from the date of issuance of the pharmaceutical practice license or from the date of the most recent certificate of completion of the training program, updating professional knowledge about pharmacy.
[6] Judicial record card, except where the judicial record database is updated and shared with the dossier-receiving agency according to the Government of Vietnam's regulations.
For foreigners or Vietnamese residing abroad, a judicial record or a document confirming they are not offenders or under criminal investigation, and not subject to the prohibition from practicing or working related to pharmaceutical activities according to a court judgment or decision issued by a competent foreign agency is required.
[7] In cases where the pharmaceutical practice license is issued due to revocation because of errors on the part of the licensing authority, the applicant only needs to submit an application form for the pharmaceutical practice license with a portrait photo of the applicant taken within no more than 6 months.
When is a pharmaceutical practice license in Vietnam reissued?
Pursuant to Article 12 of Law on Pharmacy 2016 (amended by Clause 7, Article 1 of Law on Pharmacy Amendment 2024), regulations on the issuance, reissuance, and adjustment of content of the pharmaceutical practice license are as follows:
Article 12. Issuance, Reissuance, and Adjustment of Content of the Pharmaceutical Practice License
- The issuance of the pharmaceutical practice license is carried out either by approval for applicants or by examination for those in need and applies to the following cases:
a) First-time applicants for a pharmaceutical practice license;
b) Individuals whose pharmaceutical practice license has been revoked pursuant to Article 28 of this Law.
The pharmaceutical practice license may only be reissued after 24 months from the date of revocation for cases revoked as stipulated in Clauses 4, 6, 10, and 11 of Article 28 of this Law.
2. Reissuance of the pharmaceutical practice license applies to cases of loss or damage.
- Adjustment of the content of the pharmaceutical practice license applies to cases where there is a change in the scope of practice, the form of issuance of the pharmaceutical practice license, or information about the person to whom the pharmaceutical practice license has been issued.
According to the above regulations, the pharmaceutical practice license is reissued in cases of loss or damage.
