Template for Malaria Surveillance Report during the Malaria Elimination Phase
Based on Section 1.1.3 of Subsection 1.1, Section 1, Part IV of the Guidelines for Malaria Surveillance and Prevention (issued together with Decision 4922/QD-BYT in 2021), which stipulates the reporting forms for malaria surveillance during the malaria elimination phase as follows:
Report Forms
- Develop a database for malaria elimination activities on the infectious disease surveillance management system
+ Report according to the prescribed forms.
+ Ensure that all newly emerging active malaria outbreaks are reported promptly.
+ Maintain and update the status of malaria outbreaks monthly.
- The content of the reports is to be implemented according to the prescribed forms in the Appendix 1 issued together with these Guidelines:
+ Form 1: Report of malaria cases and investigation of malaria cases.
+ Form 2: Report on the investigation and response to malaria outbreaks.
+ Form 3: Monthly report on malaria prevention and elimination activities at the commune level.
+ Form 4: Monthly report on malaria prevention and elimination activities at the district level.
+ Form 5: Monthly report on malaria prevention and elimination activities at the provincial level.
