This October, I plan to submit an application for a pharmacy practice certificate. However, I have a question: according to current regulations, do I need to submit a certified copy of the health certificate from the medical facility?

According to Article 4 of Decree 155/2018/ND-CP, annulment of the following regulations of the Government’s Decree 54/2017/ND-CP dated May 08, 2017 providing guidelines on some articles on methods for implementing the Law on Pharmacy is as follows:

1. Clause 1c and Clause 1g, Article 3.

Refer to Article 3 of Decree 54/2017/ND-CP detailing the application for the pharmacy practice certificate mentioned in Article 24 of the Law on Pharmacy consists of:

The original copy or certified true copy of the health certificate issued by a medical facility in accordance with the Law on Medical examination and treatment;

Thus, according to current regulations in Vietnam, application for a pharmacy practice certificate has abolished the submission of the original or certified copy of the Health Certificate issued by the medical facility. Therefore, in your application for a pharmacy practice certificate, you do not need to submit a health certificate.

What are procedures for converting content of a university degree in a pharmacy practice certificate in Vietnam?

According to the law, I have now completed college and received a pharmacy practice certificate. Now that I have just finished my university degree, is there a procedure to convert my pharmacy practice certificate from a college degree to a university degree?

Procedures as prescribed in Article 5 of Decree 54/2017/ND-CP, specifically, an application for adjustment to the pharmacy practice certificate mentioned in Article 26 of the Law on Pharmacy consists of:

a) The application form No. 05 in Appendix I enclosed herewith, 02 portrait pictures (4x6 cm) of the applicant on a white background which was taken within the last 6 months;

b) If the applicant’s information has to be adjusted, documents proving the adjustment which are one of the following documents: ID card, passport, family register or a confirmation issued by a competent authority as prescribed by law;

c) If the applicant’s scope of practice has to be adjusted, documents proving the adjustment: relevant qualifications, certificate of internship as an appropriate pharmaceutical facility.

In addition, there are originals or certified copies of documents in case of changes in personal information of practitioners and in cases of changes in the scope of professional activities.

How many copies is a pharmacy practice certificate in Vietnam made into?

Let me ask, is it correct that a pharmacy practice certificate can only be made into one copy and issued to a qualified person?

Clause 8, Article 6 of Decree 54/2017/ND-CP has the following provisions:

8. The pharmacy practice certificate will be made into 02 copies. One of them will be given to the applicant, the other retained by the issuing authority.

Thus, the Pharmacy Practice Certificate is made into 02 copies: 01 copy for the applicant, 01 copy is kept at the issuing authority. But you can request an additional Practice Certificate in Vietnam.
