Group of Subjects Submitting Feedback Not Eligible for Review/Denied Processing of COVID-19 Vaccination Information Feedback?
Based on Section 4 of the Appendix on the process of receiving and responding to public feedback on the COVID-19 vaccination management platform (attached to Official Dispatch 9458/BYT-CNTT in 2021), the process for receiving and responding is stipulated as follows:
Groups of individuals whose feedback is not approved/rejected for processing concerning vaccination information include:
- Photograph of the pre-vaccination screening form that lacks information on the vaccination date, dose, and vaccine type;
- Feedback information that conflicts with certification regarding the full name;
- Only personal photos sent instead of vaccination certificates;
- Mismatch between the reported dose on the Portal and the information on the vaccination certificate;
- Mismatch between the date of birth on the certificate and the date of birth on the system;
- Photographed confirmation paper lacking information on full name, gender, vaccination date, and vaccine type;
- Photographed confirmation paper but with personal information obscured;
- Blurred photographs making the text unreadable;
- Mismatch between the vaccination date on the Portal and the date on the vaccination certificate;
- Complete mismatch between all information on the certificate and the reported information;
- Incomplete or non-accented input of full names on the portal;
- Discrepancy between the reported vaccine type and the type on the certificate;
- Modified personal information on the form;
Note: In the event of remaining errors, requests for supplementary information, or rejection of feedback approval, the public can check the results on the Information Portal and will be notified to return to the vaccination facility to provide feedback and receive updated accurate information.
