At Section I, the process of verifying information and administering Covid-19 vaccination (Attached to Official Letter 8938/BYT-DP in 2021) specifies the following at the local level:
Step 1: Basic investigation before vaccination planning
The Chairman of the People's Committee of the commune, ward, town (commune level) directs the Commune Health Station to coordinate with the Commune Police (in conjunction with basic investigation of individuals in the age group of Citizen Identity Card), neighborhood groups, COVID community teams... to list (by receiving direct vaccination registration information, through data on the Covid-19 vaccination management platform self-registered by the people or registered by organizations) those in need of the 1st dose, preparing for the 2nd dose, sending to the Commune Police for verification and confirmation of people's information.
Step 2: The Commune Police will cross-check citizens' information in the population data to request citizens to supplement, complete information and then confirm and send back to the Commune Health Station for management. No verification is carried out for cases that are not Vietnamese citizens.
For cases where individuals in the list have no information or inaccurate information in the population data, the Commune Police are responsible for additional verification and sending back the list to the Commune Health Station.
The list must be sent back after verification within 02 days from the date the Health Station lists.
Step 3: Based on the verified data from the Police, the Commune Health Station will plan and organize vaccinations for the subjects according to the guidance of the Ministry of Health (prioritizing the general vaccination plan, combining the issuance of Citizen Identity Card and Covid-19 vaccination, to facilitate the people).
Individuals coming for vaccination must bring their Citizen Identity Card or notify their personal identification number with a QR code issued by the Police and the certificate of previous Covid-19 vaccination (if any).
The vaccination point checks, cross-checks the information, updates, supplements the vaccination information (if any), carries out the vaccination, enters information about the new vaccination dose on the Covid-19 vaccination management platform.
Step 4: After completing data entry for the vaccinated individual, the Chairman of the Commune People's Committee signs a certificate for the vaccination results on the Covid-19 vaccination management platform. In the absence of a certificate, the Chairman of the Commune People's Committee presides over the drafting of a record between the relevant parties to confirm the accuracy of the vaccination results data.
