Request form for amendment and supplementation of the Decision on the allocation of marine areas according to Form No. 04 issued with this Decree.
The Decision on the allocation of marine areas already granted (original copy).
A copy of the document from the competent state authority as stipulated by the law reflecting the contents specified in Clause 1 of Article 13 of this Decree.
