What Should Be Done to Correct the Mother's Name in the Birth Certificate?
Based on the provisions of Point a Clause 3 Article 2 of Circular 17/2012/TT-BYT:
In the event that a Birth Certificate was issued and an error is discovered in the recorded information, the father, mother, or a relative of the child must submit a Request for Reissuance of the Birth Certificate as per the form stipulated in Appendix 03 issued with this Circular, accompanied by documents proving the error, to the medical facility that initially issued the Birth Certificate. Within 02 working days from the receipt of the valid documents, the medical facility is responsible for retrieving the erroneous Birth Certificate to cancel it, and the request form and evidentiary documents will be archived together with the old records at the medical facility. The reissued Birth Certificate must clearly indicate the number and book number of the old Birth Certificate and be stamped "Reissued". In cases where verification is needed, the verification period shall not exceed 03 working days.
Therefore, in the event that after the Birth Certificate is issued, you discover that the mother's information on the certificate is incorrect, you must submit a Request for Reissuance of the Birth Certificate as stipulated above to request the hospital to reissue the Birth Certificate with the correct information.
Note: for errors concerning the mother's name, a photocopy of the mother's Identity Card or Citizen Card (bring the original for verification) must be attached.
