Latest 2024 Form for Application for Issuance of Certified Extracts of Civil Status Records?
According to Circular 04/2024/TT-BTP, the form for application for issuance of certified extracts of civil status records is prescribed.

How to fill in:
(1) Enter the name of the authority requesting the issuance of the certified extract of the civil status record.
(2) Only fill in if the person requesting civil status registration does not have/is not providing a personal identification number/ID card/citizen identification card/national identity card.
In cases where the information "Place of residence" must be provided, record the place of permanent residence; if there is no place of permanent residence, record the place of temporary residence; if there is no place of permanent or temporary residence, record the current place of residence.
(3) Enter the personal identification number/ID card/citizen identification card (example: Citizen Identification Number 025188001010 issued by the Department of Police Administration of Social Order on November 20, 2021). If there is no personal identification number/ID card/citizen identification card, enter a valid substitute document (passport, national identity card, etc.).
(4) Clearly state the type of civil status matter previously registered for which the issuance of a certified extract is now requested.
(5) Clearly state the name of the authority that previously registered the civil status.
Example:
- People's Committee of Dien Bien Ward, Ba Dinh District, Hanoi City.
- People's Committee of Hoan Kiem District, Hanoi City.
- Department of Justice of Hoa Binh Province.
(6) In cases where the requester cannot provide information at point (5), additional information should be supplied to aid in the search.
Example: Provide information about the father, mother to assist in the issuance of a Certified Birth Certificate; provide information about the spouse to assist in the issuance of a Certified Marriage Extract, etc.
(7) Clearly state the number of copies requested.

Can a person authorize someone else to request the issuance of a certified extract of civil status records?
According to Article 6 of the Civil Status Law 2014, there are regulations concerning individual rights and obligations for civil status registration as follows:

- Vietnamese citizens, and stateless persons residing permanently in Vietnam have the right and obligation to register civil status.
This regulation also applies to foreign citizens residing permanently in Vietnam, except in cases where international treaties of which Vietnam is a member provide otherwise.
- In cases of marriage, recognition of parents and children, the parties must directly carry out the procedure at the civil status registration authority.
For other civil status registration matters or issuance of certified extracts of civil status records, the requester may directly perform or authorize another person to perform. The Minister of Justice shall provide detailed regulations on authorization.
- Minors and legally incapacitated persons shall request civil status registration or issuance of certified extracts of civil status records through their legal representatives.
Thus, individuals can authorize someone else to request the issuance of a certified extract of civil status records.
What are the methods to request the issuance of certified extracts of civil status records?
According to Article 9 of the Civil Status Law 2014, the methods for requesting and receiving civil status registration requests are as follows:

1. When requesting civil status registration or issuance of certified extracts of civil status records, individuals shall submit the application directly to the civil status registration authority or send the application via postal system or online civil status registration system.
When performing the procedures for civil status registration or issuance of certified extracts of civil status records from the Civil Status Database, individuals shall present documents proving their identity to the civil status registration authority. If sending the application via the postal system, a certified copy of the identity documents must be sent along.
For civil status registration matters with specified processing times, the receiver of the application must issue a receipt; if the application is incomplete or invalid, the receiver must provide written guidance for the applicant to supplement and complete the application. The written guidance must specify the required additional documents clearly.
In cases where the requester submits the application to the wrong civil status registration authority, the receiver must guide the requester to the competent authority to submit the application.
Thus, the methods for requesting the issuance of certified extracts of civil status records include:
- Submitting the application directly to the civil status registration authority;
- Or sending the application via the postal system;
- Or via the online civil status registration system.
