- Application form for the Certificate of Eligibility to Trade, Import Veterinary Drugs as prescribed in Appendices XX, XXXI issued with Circular 13/2016/TT-BNNPTNT

- Detailed explanation of facilities and technical infrastructure for trading, importing veterinary drugs as prescribed in Appendix XXII issued with this Circular

- Certificate of enterprise registration or Certificate of business registration;

- Veterinary Practice Certificate.
