Request for Reissuance of a Class C Driver's License: Is a Health Certificate Required?

Based on the provisions in Clause 2, Article 36 of Circular 12/2017/TT-BGTVT, the application for reissuing a valid driver's license includes the following documents:

- Application for reissuing a driver's license according to the form specified in Appendix 19 issued with Circular 12/2017/TT-BGTVT;

- Original dossier matching the driver's license (if any);

- Driver's health certificate issued by a competent medical facility as prescribed, except for reissuing permanent driver's licenses of categories A1, A2, A3;

- Copy of identity card or citizen identification card or valid passport containing the identity card or citizen identification card number.

According to the above provisions, in your case of applying for reissuing a valid category C driver's license, you must have the driver's health certificate.
