Pursuant to Appendix II of the Circular 15/2022/TT-BGDĐT (takes effect from 24/12/2022) stipulating the sample of the Certificate of satisfaction of high school cultural knowledge requirements in Vietnam as follows:

(*) Name of educational institution that teaches high school cultural knowledge.
(1) Enter the full name of the certificate holder according to the birth certificate.
(2) Enter the date of birth according to the birth certificate. If the date of birth is from the 1st to the 9th abnd the month of birth is January or February, add a 0 in front of that number; write the year of birth in full 04 digits (Example: 07/02/2005).
(3) Enter the name of the province or affiliated cities according to the birth certificate.
(4) Enter “Male” or “Female” according to the birth certificate.
(5) Record according to the birth certificate.
(6) Enter the subjects of high school cultural knowledge according to groups of professions and occupations of vocational education.
(7) Enter the name of the educational institution that teaches high school cultural knowledge.
(8) Enter the name of the province or city directly under the Central Government where the head office of the educational institution organizing the teaching is located.
(9) Enter the date of issue of the certificate.
(10) Heads of educational institutions organize the teaching of high school cultural knowledge; Sign, write full name and seal as prescribed.
(11) Recorded by the printing agency.
(12) The certificate shall be recorded in the original certificate by the educational institution.
