INSTRUCTIONS FOR USING STUDENT MONITORING AND ASSESSMENT BOOK
1. Student monitoring and evaluation book is a record of teachers' management of teaching and educational activities, prescribed in the Center's Regulations on organization and operation.
2. Student monitoring and evaluation books are managed and used by subject teachers.
3. The teacher directly records in the notebook with all necessary information as prescribed, matching the information in the student monitoring and evaluation book (by class) of the subject/class subject to the teacher's responsibility. duties as assigned by the center. Particularly in the Comment column on the students' progress, outstanding advantages, and major limitations, teachers can choose to write in such a way that they have enough necessary information to provide their homeroom teachers according to regulations.
4. Do not write in red ink (except for repair), erasable inks. The recording of student monitoring and evaluation books must be on schedule according to the teaching plan of the professional group and carefully and cleanly preserved and maintained.
5. When repairing, use a red pen to strike out the old content, write the new content in the upper right of the old content, sign the confirmation of the repair next to the corrected content.

INSTRUCTIONS FOR USING STUDENT MONITORING AND ASSESSMENT BOOK
1. Student monitoring and evaluation books (by class) are specified in the Regulation on organization and operation of continuing education centers, vocational education and continuing education centers.
2. Student monitoring and evaluation book (by class) managed and used by the center.
3. The subject teacher directly records in the Student Monitoring and Evaluation Book (by class) full of necessary information about the subject by the teacher in charge, matching the information in the Monitoring and Evaluation Book (of the teacher), sign and write their full name at the end of the list of students for each subject. In case there are many teachers participating in teaching, the subject teachers must sign and write their full names at the end of the list of students for each subject.
The homeroom teacher directly records in the notebook for monitoring and evaluating students (according to the class) the information under the assigned tasks for the class teacher.
4. Do not write in red ink (except for repair), erasable inks; the recording of student monitoring and evaluation books (by class) must be updated on schedule according to the teaching and education plan of the professional group and preserved, kept carefully and clean.
5. When repairing, use a red pen to strike out the old content, write the new content in the upper right of the position where the old content is written, and sign the confirmation of the repair right next to or in the Notes column.
6. Centers, class teachers, and subject teachers only provide information about students' training and learning results in the student monitoring and evaluation book (by class) for each individual student or student's parents.
