What are procedures for application for re-issuance of diplomas and certificates by the Department of Education and Training in Ho Chi Minh City?

Pursuant to Clause 3, Article 16 of the Regulation on preservation, storage, use and issuance of diplomas and certificates of the national education system at the Department of Education and Training of Ho Chi Minh City issued together with the Decision 349/QD-SGDDT in 2023 stipulating procedures for re-issuance of diplomas and certificates as follows:

3. Procedures for re-issuance of diplomas and certificates are as follows:

a) The applicant for re-issuance of diplomas or certificates shall send directly or by post to the Department of Education and Trainin a set of documents including: application for re-issuance of diplomas and certificates; diplomas and certificates requested for re-issuance; papers proving that the Department of Education and Training granted diplomas or certificates incorrectly;

b) In case the form of diploma or certificate at the time of granting the diploma or certificate has changed, the Department of Education and Training shall issue the diploma or certificate using the current form of the diploma or certificate to issue to the person who is re-granted the diploma or certificate.

Therefore, the procedures for re-issuance diplomas and certificates at the Department of Education and Training of Ho Chi Minh City are as follows:

Step 1: The person who requests the re-issuance of the diploma or certificate shall send a set of documents directly or by post to the Department of Education and Training for re-grant of the diploma or certificate including:

+ An application for re-issuance of diplomas and certificates;

+ Diplomas and certificates requested for re-issuance;

+ Papers proving that the Department of Education and Training granted diplomas or certificates incorrectly;

Step 2: The Department of Education and Training re-issues diplomas and certificates.

In case the form of diploma or certificate at the time of granting the diploma or certificate has changed, the Department of Education and Training shall issue the diploma or certificate using the current form of the diploma or certificate to issue to the person who is re-granted the diploma or certificate.

What are responsibilities of the Department of Education and Training of Ho Chi Minh City?

Pursuant to Article 5 of the Regulation on preservation, storage, use and issuance of diplomas and certificates of the national education system at the Department of Education and Training of Ho Chi Minh City issued together with the Decision 349/QD-SGDDT in 2023 stipulating responsibilities of the Department of Education and Training of Ho Chi Minh City as follows:

- The Department of Education and Training that grants diplomas and certificates is responsible for:

+) Check, compare and record accurately and fully all information in diplomas and certificates. Request the diploma or certificate grantee to confirm that the information has been checked, to confirm the information correction requests and to provide the grounds for the information correction request;

+) Compile complete dossiers for granting, managing diplomas and certificates and storing them according to the provisions of this Regulation;

+) Ensure the accuracy and take responsibility for the information in the original book of granting diplomas and certificates;

+) Ensure physical foundations, equipment, fire and explosion prevention to preserve diplomas, certificates and documents for issuance and management of diplomas and certificates.

The director of the Department of Education and Training that grants diplomas and certificates is responsible for:

+) Promulgate regulations on preservation, storage, use and issuance of diplomas and certificates of the Department of Education and Training. Regulations must clearly define responsibilities of each unit and individual and sanctions for handling violations when violations occur;

a) Print and issue diplomas and certificates according to their competence and on time as prescribed;

+) Revoke or cancel diplomas and certificates according to regulations;

a) Issue copies of diplomas and certificates from the original books as prescribed;

+) Assign under orders to Head of Examination and Education Quality Accreditation Department to sign and issue Official Dispatch replying to results:

++ Authenticate diplomas and certificates at the request of agencies, organizations and individuals;

++ For cases that are not eligible for recognition of diplomas issued by foreign educational institutions for use in Vietnam.

+) Edit the content of diplomas and certificates according to regulations; re-grant diplomas and certificates according to regulations;

+) Take personal responsibility before the superior management agency and before the law for the management and issuance of diplomas and certificates.
