Procedure for Re-issuance of a Lost Copy of High School Graduation Certificate
Based on Article 31 of the Graduation Certificate Management Regulations issued together with Circular 21/2019/TT-BGDDT, the procedures are as follows:
- The individual requesting the issuance of a copy of the diploma or certificate shall directly submit or send via postal mail one (01) set of documents requesting the issuance of a copy of the diploma to the competent authority.
The dossier includes:
- A request form for the issuance of a copy of the diploma or certificate, providing information about the previously issued diploma or certificate;
- The original or a certified copy of the identity card, citizen identification, or passport that is valid for the recipient to verify;
- In the event that the individual requesting the issuance of a copy of the diploma or certificate from the original register is the person stipulated in clauses 2 and 3 of Article 30 of these Regulations, a power of attorney (for the authorized person) or documents proving the relationship with the holder of the original diploma or certificate must be presented;
- In the case where the individual requesting the issuance of a copy of the diploma or certificate from the original register sends the request via postal mail, they must send a certified copy of the documents specified in items a, b, and c of this clause along with one (01) stamped envelope clearly stating the full name and address of the recipient to the competent authority for issuing copies of the diploma or certificate from the original register.
On the same day of receiving the request for issuing copies or on the next working day, if the request is received after 3:00 PM, the competent authority issuing copies of the diploma or certificate from the original register must issue the copies to the requesting individual. In the case where the request for issuing copies from the original register is sent via postal mail, the time limit is determined from the time the competent authority receives the complete and valid dossier according to the postal mark;
For cases where multiple copies from various original registers are requested at the same time, a large quantity of copies is required, or the content of the diploma or certificate is complex and difficult to verify, and the receiving authority cannot meet the deadline stipulated in item b of this clause, the issuance time may be extended by no more than two (02) additional working days or longer as agreed in writing with the individual requesting the copies.
