2 Types of contributed assets include:
- Vietnam Dong
- Freely convertible currency (type of foreign currency and contributed amounts are specified)
- Gold
- Value of land use right, intellectual property rights, technology or technical know-how
- Other assets (type of assets, quantity and their residual value are specified, a list of assets may be enclosed with this application form)

- In case of application for enterprise registration, date of capital contribution is the date on which the member completes the capital contribution.
- In case of application for registration of increase of charter capital, date of capital contribution is the date on which the member completed the capital contribution.
- In other cases, the date of capital contribution is not compulsory.
4 This information is required in case of application for enterprise registration. In case a member is an organization, the member’s signature is the one of the legal representative of that organization.
5 This form must be personally signed by the company’s legal representative.
