1 In case the Certificate of branch/ representative office/ business location registration is granted after the commencement of declared business lines, the business commencement date shall be the date of issue of the Certificate of branch/ representative office/ business location registration.

2 In case an accounting year is the calendar year, specify from January 01 to December 31. If an accounting year is the financial year other than the calendar year, specify the date of beginning of the accounting year which is the beginning date of the quarter, and the date of ending the accounting year which is the last date of the quarter; Total period from the beginning to the ending of an accounting year must be a full period of 12 consecutive months or 04 consecutive quarters.

3 Pursuant to the laws on VAT and based on its business plan, the enterprise choose one of 04 VAT calculation methods in this item.

4 The person competent to sign enterprise’s notifications as regulated in Point e Clause 2 Article 33 of the Decree No. 78/2015/ND-CP, Clause 2 Article 6 of the Circular No. 20/2015/TT-BKHDT shall personally write his/her signature in this part.
