Application for Reissuance of Certificate of Eligibility for Food Safety

According to Clause 11 Article 10 of Decree 17/2020/ND-CP (Effective from March 22, 2020) stipulated as follows:

a) In case of application for reissuance of the Certificate due to loss or damage

An application form as specified in Form No. 01b, Section I, Appendix issued together with this Decree.

b) In case of application for reissuance of the Certificate due to changing the production, business location; changing, supplementing the production process, business items or when the Certificate expires:

- An application form as specified in Form No. 01b, Section I, Appendix issued together with this Decree;

- Dossier as stipulated in Clause 1, Article 12 of this Decree.

c) In case of application for reissuance of the Certificate due to changing the name of the facility but not changing the owner of the facility, address, location, and the entire production process, business items:

- An application form for issuance as specified in Form No. 01b, Section I, Appendix issued together with this Decree;

- A copy of the Certificate of eligibility for food safety issued (certified by the owner of the facility);

- Documents proving the change of the facility name.

d) In case of application for reissuance of the Certificate due to changing the owner of the facility but not changing the name of the facility, address, location, and the entire production process, business items:

- An application form for issuance as specified in Form No. 01b, Section I, Appendix issued together with this Decree;

- A copy of the Certificate of eligibility for food safety issued (certified by the facility);

- A summary list of health certificates certified by the facility owner or health certificates of the facility owner and the person directly involved in food production, business issued by a district-level or higher medical facility (certified by the facility).
