Template Request for Conversion of Construction Practice Certificate
Clause 14 Article 1 Decree 100/2018/ND-CP stipulates the dossier for the request to convert construction practice certificates, including:
- Application for the conversion of construction practice certificates as per the form in Appendix III of this Decree, attached with 02 color photos sized 4 x 6 cm with a white background of the applicant taken within no more than 06 months;
- A copy of the training degree, practice certificate issued by a foreign agency or organization that has been consular legalized, translated into Vietnamese, and notarized or authenticated according to the laws of Vietnam;
- A certified copy or a file containing color images taken from the original or the copy, presenting the original for comparison of legal documents regarding residence or work permits issued by Vietnamese competent authorities as prescribed.
According to this regulation, the Application for the conversion of practice certificates should follow the form in Appendix III of Decree 100/2018/ND-CP. Specifically, as follows:
