Does the Application for Issuance of Class III Construction Practice Certificate Require a Health Certificate?
According to Clause 1, Article 76 of Decree 15/2021/ND-CP prescribing the application for the issuance of a practicing certificate in construction activities, including:
The application for initial issuance, certificate grade adjustment, certificate renewal includes:
- Application form for issuance of a practicing certificate according to Form No. 01, Appendix IV of this Decree;
- Two 4 x 6 cm color photos and a photo file with a white background portrait of the applicant taken within the last 06 months;
- A diploma issued by a legal training institution in accordance with the type and grade of the requested certificate;
For diplomas issued by a foreign training institution, the diploma must be consular legalized as prescribed and must have a Vietnamese translation certified and authenticated as prescribed by Vietnamese law;
- Practicing certificate already issued by the competent authority in case of requesting certificate grade adjustment or renewal;
- Assignment decisions (task assignment) of the organization for the individual or a confirmation document from the legal representative of the investor regarding the typical works that the individual has completed as declared. The signatory must be responsible for the truthfulness of the confirmation content. In the case of freelance individuals, there must be a contract and an acceptance record of the declared typical works;
- Legal documents regarding residency or a work permit issued by the competent authority of Vietnam for foreign individuals;
- A copy of the examination results meeting the requirements in case the examination was conducted before the application submission date;
- The documents specified in points c, d, dd, and e of this clause must be certified copies or files containing color photos taken from the original or a copy, presenting the original for comparison.
Thus, the application for the issuance of a practicing certificate in construction activities does not mandatorily require a Health Certificate. Therefore, you only need to prepare the full documents as prescribed above.
