Authorization Form for Investment in Specialized Aviation Services Projects
Based on Chapter III Appendix III issued together with Circular No. 23/2021/TT-BGTVT (Effective from December 25, 2021) stipulating the form of power of attorney in investing in aviation service projects as follows:

I am [name, ID/passport number, title of the legal representative of the investor], the legal representative of [name of the investor] located at [address of the investor], hereby authorize: [name, ID/passport number, title of the authorized person] to perform the following tasks during the process of applying for the investor approval for the project [name of the project] organized by the Civil Aviation Authority of Vietnam:
[The scope of authorization includes one or several of the following tasks:
- Signing the application for investor approval;
- Signing the partnership agreement (if any);
- Signing documents and transactions with the Civil Aviation Authority of Vietnam during the process of applying for investor approval, including requests for clarification of the project proposal, explanatory documents, clarifications of the project proposal;
- Participating in the process of negotiating and finalizing the contract;
- Signing the contract with the competent State agency if the investor is approved.]
The authorized person mentioned above shall only perform tasks within the scope of authorization as the legal representative of [name of the investor]. [name of the legal representative of the investor] shall take full responsibility for the work carried out by [name of the authorized person] within the scope of authorization.
This power of attorney is effective from to ^(1)^. This power of attorney is made in copies, all of which are legally valid. The principal keeps one copy. The authorized person keeps copies. Attach one (01) original copy to the original of the application for investor approval.

Note:
(1) Indicate the effective date and expiration date of the power of attorney in accordance with the bidding process.
(2), (3) Use of the seal in case of authorization can be the seal of the investor or the unit to which the individual involved is authorized.
